Participation sought for health needs assessment, health improvement plan

Veterans Memorial Hospital Community and Home Care, formerly Allamakee County Public Health, would like to invite anyone in the Allamakee County community to take a survey for Community Health Needs Assessment (CHNA) and Health Improvement Plan (HIP) assessment.  This plan is a report documenting a community-wide effort to assess Allamakee County’s health needs and decide how to meet them.
Community and Home Care has been working on this for several months now.  The plan is updated at least every five years. It is very important that community members participate in making decisions about improving the public’s health. In order to see what issues need to be focused upon Community and Home Care staff are asking community members to take several minutes and go to the Veterans Memorial Hospital website at www.veteransmemorialhospital.com and complete this survey,  call 563-568-5660 to have a copy of the survey mailed to you or stop in and fill out the survey in the Community and Home Care office, which is located on the third floor of Veterans Memorial Hospital in Waukon. All surveys are completely confidential and non-identifiable.
Community and Home Care has been offering these surveys at various locations around the county such as the Allamakee County Fair, Waukon Corn Days meal, and the area school registrations. Participation in this process can help Allamakee County to receive benefits such as grant money or initiate other needed health services in the local area.
An accurate evaluation of Allamakee County’s health needs will allow the State of Iowa to look at what kind of health improvements the community needs. The reports created from this survey will provide guidance on needed resources and address health priorities for the community.
