Veterans Memorial Hospital in Waukon will be opening the new Waukon Clinic Monday, May 3. Those wishing to make an appointment are invited to call the new clinic number of 563-568-5530 now to schedule an appointment for May or beyond.
Health insurance plans that are currently contracted with Veterans Memorial Hospital will also be accepted in the clinic. Those who are questioning if their health plan will be accepted should call their insurance company. Medicaid covered patients should contact Medicaid to make Veterans Memorial Hospital Waukon Clinic their primary care facility.
The hospital’s electronic medical record system will also be integrated into this new clinic with access to electronic records from other medical facilities.
Dr. Dave Schwartz will be the medical physician provider in this new clinic. Two nurse practitioners are also joining the team, Sue McMillan, ARNP and Halie Ruff, ARNP. In addition, Jon Hawes, PA and current Emergency Provider at the hospital will also begin seeing patients in the clinic in July, on a part-time basis. Three medical assistants have been recruited as well to work with these providers. Laboratory and X-ray services will be provided by the existing hospital departments.
The hospital conducted a needs analysis prior to making the determination to open this clinic that showed an overwhelming need for more providers in the area. The plans are to start with these providers and expand the family practice providers as well as specialty providers in the near future.
The Veterans Memorial Hospital Waukon Clinic will be open Monday through Friday 8 a.m. to 5 p.m. with a special “sick child” clinic every Monday morning starting at 7 a.m. Evening appointments will also be offered Thursdays until 7 p.m. The hospital’s urgent care clinic will also remain available Monday through Friday from 5-7 p.m. and weekends and holidays 9 a.m. to 1 p.m. for use when the clinic is closed.
